📖 How to Use This Template

1

Download the CSV

Click the download button above. Save the file to your computer or phone. No account, no email — the file downloads instantly.

2

Open in your spreadsheet app

Double-click to open in Excel, Google Sheets, Numbers, or LibreOffice. All 12 columns are pre-labeled with headers — just start filling in rows.

3

Enter a bill each time you get one

Fill in provider name, date of service, and billed amount from the bill. Then cross-reference against your EOB for the insurance paid and patient responsibility amounts.

4

Compare bill vs. EOB

If the billed amount minus insurance paid doesn't match what you're being asked to pay, flag it in the dispute column. Common errors: duplicate billing, upcoding, out-of-network charges at in-network facilities.

5

Track your deductible progress

As you enter each bill, update the deductible impact column. When the total reaches your plan's deductible, your insurance coverage changes — know exactly when that happens.

6

Mark payments and keep for HSA records

When you pay a bill, mark the date and status. These records are essential for HSA reimbursement — you can reimburse yourself from your HSA years later, tax-free.
